Download the declaration sheet here. After downloading print it and cut the sheet out along the broken line and send it attached to your declaration of income for tax return to the NAV (National Tax and Customs Administration) in a closed envelope, on which your tax identification number is given.

The deadline of submission: 20th may 2012 

If the declaration is submitted to the employer, the deadline of submission is determined by the employer. (However, if someone cannot keep the deadline, it is possible to send the declaration until the 21th of May to the tax authorities in a sealed envelope on which your tax identification number is given.)
